We are glad that you have decided to respond to the survey electronically.
Before we start, you will need your NSNA-ID. This can be found on the label of the mail you received or on page 2 of the survey mailing. Once you locate that, you are all set to start the survey responses.
You will need Adobe Acrobat Reader and an email account configured on your machine. If you don't have email account configured, you can fill in the form and print the form and mail it.
There are two essential parts to the survey.
- Membership Information Update: You will get a chance to update missing information from page 2 of the survey mailing.
- Membership Opinion: You will get a chance to tell us what you think about some of the topics that we have listed, plus a general comment section to give us your opinions.
